Souvenir T-Shirt Order Form - 2010

High quality gold T-shirts with the 2010 insignia, “Celebration of Champions” on front.  Tournament location, dates, national map, and list of participating churches will also be on shirt.  Teams must be registered by May 25 in order to be listed on the shirts.  Please order and pay by May 25, 2010 in order to reserve your shirts!!!  After May 25^th, please contact Phil Walton (see information below) to check availability.  Child 14-16 is the smallest size that we can offer due to the size of the information that we print on the back of the shirts.  We are sorry that we cannot offer the smaller shirts.

DO NOT ASSUME THAT AN EMAIL HAS BEEN RECEIVED OR THAT AN ORDER HAS BEEN RECEIVED FROM THE WEBSITE.  I WILL REPLY WITH CONFIRMATION TO ALL EMAIL ORDERS AND TO ALL WEBSITE ORDERS.
